A formerly active 68 year old woman awoke from surgery paralyzed following an incorrect spinal surgery. After the surgery, no MRI was done to diagnose and correct the problem. The surgeon then placed an order in the medical record that no MRI be performed, despite the neurological emergency. Three months later an MRI revealed a mass compressing her spine. A subsequent surgery was done, and after that surgery the mass and compression were no longer present. That surgery worsened her condition.
